Responsibilities
- Conduct thorough patient assessments via phone or telehealth platforms to determine the urgency of medical needs.
- Evaluate symptoms, review medical history, and recommend appropriate treatment plans or referrals to specialists.
- Collaborate seamlessly with physicians and care coordinators to ensure a continuum of high-quality patient care.
- Maintain high call volume efficiency while adhering to strict quality assurance and HIPAA compliance standards.
- Provide patient education on preventative health measures, medication management, and self-care strategies.
- Document all clinical interactions accurately and timely in the Electronic Health Record (EHR).
- Escalate critical cases immediately to on-call medical providers.
Qualifications
- Active and unencumbered Registered Nurse (RN) license in the State of Texas.
- Minimum of 2-3 years of acute care, emergency room, or urgent care nursing experience.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Strong analytical and critical thinking skills with the ability to make rapid, sound clinical decisions.
- Exceptional verbal and written communication skills for remote patient interaction.
- Proficiency with electronic health record (EHR) systems such as Epic or Cerner.
- Compassionate demeanor with the ability to manage high-pressure situations effectively.
