Responsibilities
- Conduct comprehensive telephone and virtual triage assessments for patients presenting with acute and chronic health concerns.
- Evaluate patient symptoms, medical history, and current medications to determine the appropriate level of care and urgency.
- Document all triage encounters accurately and in real-time within the electronic health record (EHR) system.
- Collaborate effectively with physicians, nurse practitioners, and other healthcare professionals to ensure continuity of care.
- Provide clear, empathetic health education and discharge instructions to patients and families.
- Identify high-risk patients and facilitate timely referrals to urgent care facilities or emergency services when necessary.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the State of New Mexico.
- Minimum of 2-3 years of clinical nursing experience, preferably in a triage or call center setting.
- Certification in Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) is required.
- Strong communication and interpersonal skills with the ability to remain calm under pressure.
- Proficiency in navigating electronic medical records (EMR/EHR) systems.
- Ability to work independently and make sound clinical judgments based on available data.
