Responsibilities
- Assess Patient Symptoms: Conduct thorough evaluations of patient complaints via phone or in-person to determine urgency and appropriate care pathways.
- Provide Triage Decisions: Utilize clinical judgment to recommend home care, office visits, or emergency department referrals.
- Coordinate Care: Facilitate timely referrals to specialists and coordinate follow-up appointments to ensure continuity of care.
- Documentation: Maintain accurate and up-to-date electronic health records (EHR) detailing all patient interactions and triage outcomes.
- Clinical Collaboration: Communicate effectively with physicians, nurse practitioners, and other healthcare professionals to ensure optimal patient outcomes.
- Patient Education: Educate patients on self-care instructions, medication management, and preventative health strategies.
Qualifications
- Licensure: Active Registered Nurse (RN) license in the state of Texas.
- Experience: Minimum of 2-3 years of experience in triage, emergency room, or urgent care nursing.
- Education: Bachelor of Science in Nursing (BSN) preferred; Associate Degree in Nursing (ADN) accepted with relevant experience.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Skills: Strong critical thinking, excellent verbal communication, and proficiency in electronic medical records systems.
- Availability: Ability to work flexible hours, including evenings and weekends, as required.
