Responsibilities
- Conduct Triage Assessments: Utilize clinical judgment to evaluate patient symptoms via phone or secure messaging to determine urgency and appropriate care pathways.
- Patient Education: Provide clear, empathetic guidance on home care management, medication instructions, and when to seek further medical attention.
- Documentation: Accurately record patient interactions, medical history, and triage outcomes in Electronic Health Records (EHR) systems in real-time.
- Collaboration: Communicate effectively with physicians, urgent care staff, and emergency services to ensure seamless patient handoffs.
- Crisis Management: Identify critical medical emergencies and initiate immediate protocols to ensure patient safety.
- Quality Assurance: Participate in quality improvement initiatives and adhere to all HIPAA and clinical protocols.
Qualifications
- Licensure: Active Registered Nurse (RN) license in the State of California required.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications are mandatory.
- Experience: Minimum of 2-3 years of experience in acute care, emergency room, or triage nursing preferred.
- Technical Skills: Proficiency with EMR/EHR systems (e.g., Epic, Cerner) and strong computer literacy.
- Communication: Excellent verbal and written communication skills with the ability to convey complex medical information clearly to patients of all backgrounds.
- Stamina: Ability to handle high-volume call volumes and maintain composure under pressure.
