Responsibilities
- Perform comprehensive telephone and in-person triage assessments to determine patient acuity and medical needs.
- Apply clinical judgment to prioritize care and determine appropriate referral routes to physicians or specialists.
- Document all patient interactions, assessments, and care plans accurately within the Electronic Health Record (EHR).
- Collaborate with physicians and nursing staff to ensure seamless patient flow and continuity of care.
- Provide patient education and instructions regarding home care, medications, and follow-up appointments.
- Respond to high-volume inquiries during peak hours while maintaining a calm and professional demeanor.
Qualifications
- Active and unencumbered Registered Nurse (RN) license in the Commonwealth of Pennsylvania.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications are required.
- Minimum of 2 years of clinical nursing experience, preferably in an urgent care, emergency room, or telehealth setting.
- Strong interpersonal and communication skills, with the ability to de-escalate patient concerns effectively.
- Proficiency in using Electronic Health Records (EHR) systems and computer applications.
- Ability to work flexible shifts, including evenings, nights, and weekends as required by the department.
