Responsibilities
- Conduct Patient Assessments: Utilize evidence-based protocols to evaluate patient symptoms via phone or secure messaging, determining the appropriate level of medical urgency.
- Facilitate Care Coordination: Collaborate with physicians, urgent care providers, and emergency departments to ensure seamless patient transitions and appropriate referrals.
- Documentation: Maintain accurate and up-to-date electronic health records (EHR) for all triage encounters, ensuring compliance with HIPAA regulations.
- Patient Education: Provide clear, empathetic instructions to patients regarding home care, medication management, and when to seek immediate medical attention.
- Resource Management: Assist in managing the flow of patient inquiries, triaging non-emergent issues to reduce unnecessary ER visits and optimize clinic capacity.
- Quality Assurance: Participate in regular case reviews and performance improvement initiatives to enhance triage protocols and patient outcomes.
Qualifications
- Licensure: Current, unrestricted Registered Nurse (RN) license in the state of Oklahoma.
- Experience: Minimum of 2-3 years of experience in emergency nursing, urgent care, or a related clinical setting preferred.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required; TNCC (Trauma Nursing Core Course) is a plus.
- Communication Skills: Superior verbal and written communication skills with the ability to de-escalate situations and explain complex medical concepts clearly.
- Technical Proficiency: Proficient in navigating electronic health records (EHR) systems and telehealth platforms.
- Work Ethic: Strong critical thinking skills and the ability to make quick, sound clinical decisions under pressure.
