Responsibilities
- Conduct comprehensive phone and digital assessments to evaluate patient symptoms and medical history.
- Make critical triage decisions to determine the appropriate level of care, including home care, urgent care, or emergency room admission.
- Document all patient interactions and clinical findings accurately in electronic health records (EHR).
- Provide patient education and self-care instructions to promote recovery and prevent readmission.
- Collaborate with physicians and other care team members to ensure continuity of care.
- Handle high call volumes while maintaining a calm, empathetic, and professional demeanor.
- Identify and escalate urgent or critical cases immediately.
Qualifications
- Active, unencumbered Registered Nurse (RN) license in the State of Indiana.
- Minimum of 2 years of recent nursing experience, preferably in a triage or urgent care setting.
- Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications.
- Strong knowledge of pathophysiology, pharmacology, and clinical decision-making protocols.
- Excellent verbal communication skills with the ability to convey complex medical information clearly.
- Proficiency in using computer systems and EMR software.
- Ability to work independently and manage time effectively in a remote or virtual environment.
