Responsibilities
- Conduct thorough patient assessments via phone, email, and in-person triage to determine acuity levels.
- Make appropriate referrals to primary care physicians, specialists, or emergency services.
- Document patient encounters accurately and in compliance with HIPAA regulations.
- Provide patient education and wellness coaching to prevent readmission.
- Collaborate with the care team to ensure seamless continuity of care.
- Manage incoming communications and triage queues efficiently during peak hours.
Qualifications
- Active Registered Nurse (RN) license in the state of Minnesota.
- Current BLS and ACLS certification.
- Minimum of 2 years of clinical nursing experience, with specific experience in triage or telephone nursing preferred.
- Strong decision-making and critical thinking skills.
- Excellent verbal and written communication abilities.
- Proficiency with electronic health record (EHR) systems.
- Ability to remain calm and professional under pressure.
