Responsibilities
- Perform initial patient assessments via phone and in-person to determine acuity and urgency of care needs.
- Apply triage protocols to prioritize patient flow and ensure the right care is provided at the right time.
- Document patient interactions, medical histories, and care plans accurately in the Electronic Health Records (EHR) system.
- Coordinate referrals to specialists, physicians, and other healthcare providers.
- Provide patient education regarding symptoms, home care, and preventative measures.
- Collaborate with the clinical team to resolve complex patient care issues.
- Maintain a professional and empathetic demeanor in high-stress situations.
Qualifications
- Active, unencumbered Registered Nurse (RN) license in Minnesota.
- Graduate of an accredited Nursing program (Associate Degree in Nursing or Bachelor of Science in Nursing).
- Strong interpersonal and communication skills.
- Ability to remain calm and make sound clinical decisions under pressure.
- Basic computer literacy and willingness to learn new software systems.
- Compassionate attitude with a focus on patient-centered care.
