What You'll Do:
- Perform comprehensive telephone and digital health triage assessments.
- Evaluate patient symptoms to determine appropriate level of care and urgency.
- Document patient interactions accurately and efficiently in the EMR system.
- Provide clear patient education and follow-up instructions.
- Collaborate effectively with physicians and care coordination teams.

Responsibilities
- Conduct detailed symptom assessments via phone and secure messaging.
- Identify urgent medical needs and facilitate timely referrals.
- Maintain strict confidentiality and adhere to HIPAA regulations.
- Monitor patient status and escalate critical issues immediately.
- Participate in continuous quality improvement initiatives.
Qualifications
- Active, unencumbered California RN License.
- Minimum of 2 years of nursing experience, preferably in a clinical or triage setting.
- Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications.
- Strong computer literacy and proficiency with EMR software.
- Exceptional communication and interpersonal skills.
