Responsibilities
- Conduct comprehensive telephone assessments of patients presenting with acute and chronic health concerns.
- Utilize clinical decision-making skills to triage patients into appropriate care pathways (e.g., home care, urgent care, ER).
- Document patient interactions, diagnoses, and care plans accurately in the EMR system.
- Educate patients and caregivers on self-care management and medication instructions.
- Coordinate referrals to specialists or other healthcare providers as needed.
- Collaborate with physicians and nurse practitioners to ensure continuity of care.
Qualifications
- Current and active RN license issued by the Massachusetts Board of Registration in Nursing.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Minimum of 2 years of acute care or emergency room nursing experience.
- Previous experience in telehealth or telephone triage is highly preferred.
- Strong computer skills and proficiency with Electronic Medical Records (EMR).
- Excellent verbal and written communication skills with a focus on patient-centered care.
