Responsibilities
- Conduct comprehensive patient assessments via phone, email, or secure video platforms.
- Utilize clinical judgment and critical thinking to triage symptoms and determine appropriate levels of care.
- Document patient interactions, medical histories, and triage decisions accurately in Electronic Health Records (EHR).
- Coordinate referrals to specialists, urgent care centers, or emergency services as necessary.
- Provide patient education, medication reconciliation, and symptom management guidance.
- Maintain strict confidentiality and adhere to HIPAA regulations at all times.
Qualifications
- Active, unencumbered Registered Nurse (RN) license in California.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Minimum of 2 years of clinical experience, preferably in a triage or telehealth setting.
- Strong proficiency with EMR/EHR systems (e.g., Epic, Cerner).
- Excellent verbal communication, active listening, and conflict resolution skills.
- Ability to remain calm under pressure and make quick, informed decisions.
