Responsibilities
- Deliver comprehensive virtual patient assessments via telehealth platform
- Coordinate care plans with multidisciplinary teams across specialties
- Provide expert patient education and medication management remotely
- Respond to critical alerts and triage urgent virtual consultations
- Maintain accurate electronic health records with precision
- Participate in continuous quality improvement initiatives
- Collaborate with inpatient teams for seamless care transitions
Qualifications
- Active WA RN license in good standing
- Minimum 2 years acute care or telehealth experience
- BLS certification with ACLS preferred
- Expertise in EHR systems (Epic/Cerner a plus)
- Exceptional critical thinking and assessment skills
- Strong communication abilities for virtual patient interactions
- Ability to work independently with minimal supervision
- Experience with remote monitoring technologies
