Responsibilities
- Deliver high-quality direct patient care, including daily hygiene, grooming, and feeding assistance.
- Observe and report vital signs, changes in patient condition, and behavioral shifts to the Registered Nurse.
- Assist patients with mobility, transfers, and ambulation while ensuring safety protocols are followed.
- Document patient care activities and clinical notes accurately and timely in the Electronic Health Record (EHR).
- Collaborate with the interdisciplinary team to develop and implement individualized care plans.
- Maintain a clean, safe, and therapeutic environment for patients and staff.
Qualifications
- Certified Nursing Assistant (CNA) license in the state of Texas (active and unencumbered).
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification.
- Previous experience in a hospital, nursing home, or home health setting preferred.
- Strong interpersonal and communication skills.
- Ability to work weekends, holidays, and rotating shifts as required.
- Physical ability to lift, push, and pull patients and equipment.
- Valid driver's license and reliable transportation.
