Responsibilities
- Provide high-quality virtual patient assessments and care coordination via telehealth platform
- Monitor and evaluate patient progress through digital health records and remote monitoring tools
- Collaborate with interdisciplinary teams to develop and implement personalized care plans
- Deliver patient education and health coaching via video consultations and digital resources
- Document clinical encounters accurately in compliance with HIPAA regulations
- Respond to patient inquiries and triage urgent situations remotely
- Participate in continuous quality improvement initiatives for virtual care delivery
Qualifications
- Active California Registered Nurse (RN) license in good standing
- Minimum 2 years of clinical nursing experience (telehealth preferred)
- Bachelor of Science in Nursing (BSN) or higher
- Proficiency with electronic health records (EHR) and telehealth platforms
- Strong critical thinking and decision-making abilities in virtual settings
- Excellent communication skills with ability to build rapport remotely
- CPR/BLS certification and relevant specialty certifications
- Ability to work independently with minimal supervision
