Responsibilities
- Provide virtual patient monitoring and health assessments via telehealth platforms
- Assist patients with daily living activities through remote guidance and support
- Document patient care records accurately in our electronic health systems
- Coordinate with medical teams to ensure seamless patient care continuity
- Respond to patient inquiries and provide immediate remote assistance
- Participate in virtual care team meetings and training sessions

Qualifications
- Active Indiana CNA certification required
- Minimum 1 year of hospital or clinical experience
- Proficiency with telehealth technology and electronic medical records
- Exceptional communication and empathy skills
- Ability to work independently with minimal supervision
- Reliable high-speed internet and dedicated home workspace
- Flexible availability including evenings and weekends
- Valid CPR certification
