Responsibilities
- Deliver high-quality virtual patient assessments and consultations
- Develop and implement personalized care plans remotely
- Coordinate with multidisciplinary healthcare teams
- Provide patient education and health coaching via telehealth platforms
- Accurately document patient encounters in EMR systems
- Monitor patient progress and adjust care plans as needed
- Handle urgent triage and emergency response protocols
Qualifications
- Active RN license in Texas (required)
- Minimum 2 years clinical nursing experience
- Telehealth or virtual care experience preferred
