Responsibilities
- Deliver comprehensive virtual patient assessments and care coordination via telehealth platforms
- Collaborate with multidisciplinary teams to develop and implement personalized care plans
- Monitor patient vitals and medication adherence through remote monitoring systems
- Provide urgent triage and intervention for acute clinical situations
- Maintain accurate electronic health records (EHR) documentation
- Conduct patient education and health coaching sessions remotely

Qualifications
- Active California RN license with no disciplinary actions
- Minimum 2 years of acute care or telehealth experience
- BLS/ACLS certifications required (PALS preferred)
- Proficiency in telehealth platforms and EHR systems

- Strong written and verbal communication abilities
