Responsibilities
- Provide high-quality patient assessments and care coordination via telehealth platforms
- Monitor patient progress and adjust care plans remotely
- Collaborate with multidisciplinary teams for integrated patient management

Qualifications
- Active RN license in Illinois (or compact state)
- Minimum 2 years clinical nursing experience
- BLS/CPR certification required
- Experience with telehealth platforms (e.g., Teladoc, Amwell)
- Strong assessment and critical thinking skills
- Excellent communication and computer literacy
- Ability to work independently with minimal supervision
- Bachelor's degree in Nursing (BSN) preferred
