Responsibilities
- Provide compassionate assistance with daily activities (bathing, dressing, mobility)
- Monitor and document vital signs, fluid intake, and dietary preferences
- Administer prescribed medications under RN supervision
- Assist with wound care and infection prevention protocols
- Transport residents safely to therapy appointments or social activities
- Maintain accurate electronic health records (EHR)
- Collaborate with interdisciplinary care teams
Qualifications
- Active Illinois CNA certification in good standing
- Minimum 1 year long-term care experience
- Current CPR/BLS certification
- Proficient in EHR documentation systems
- Ability to lift 50+ pounds with proper techniques
- Clear criminal background check and TB screening
- Strong communication and problem-solving skills
