Responsibilities
- Provide direct patient care including bathing, dressing, and grooming assistance.
- Monitor and record vital signs (blood pressure, temperature, pulse) accurately and timely.
- Assist with mobility, transfers, and ambulation using proper body mechanics and assistive devices.
- Document patient care plans, incidents, and changes in condition in electronic health records.
- Communicate effectively with residents, families, and the nursing team to ensure holistic care.
- Maintain a clean, safe, and sanitary environment for residents.
- Administer medications and treatments as prescribed by the RN or LPN under supervision.
Qualifications
- Current and active Illinois CNA license (CNA I or CNA II).
- CPR/BLS certification (Basic Life Support) required.
- High School Diploma or GED equivalent.
- Previous experience in a long-term care or hospital setting preferred.
- Strong communication and interpersonal skills.
- Ability to stand for extended periods and lift patients (up to 50 lbs).
- Compassionate attitude and commitment to patient dignity and respect.
