Responsibilities
- Provide direct patient care and assistance with Activities of Daily Living (ADLs) including bathing, dressing, and grooming.
- Monitor and record vital signs (temperature, blood pressure, pulse) accurately and report any abnormalities to the RN/Charge Nurse.
- Administer medications and treatments as prescribed by the physician, following all safety protocols.
- Maintain a clean, safe, and sanitary environment for patients to prevent infection and promote comfort.
- Document patient information and care provided accurately in the electronic medical record (EMR) system.
- Communicate effectively with patients, families, and the interdisciplinary care team to ensure continuity of care.
- Assist with patient transfers and mobility using proper body mechanics and equipment.
Qualifications
- Certified Nursing Assistant (CNA) license in good standing (Ohio required).
- Current Basic Life Support (BLS) Certification from the American Heart Association.
- High School Diploma or GED equivalent.
- Previous experience in a hospital, long-term care, or home health setting is preferred.
- Strong interpersonal and verbal communication skills.
- Ability to work weekends (Friday through Sunday) and rotating shifts.
- Compassionate, patient-centered approach with a strong attention to detail.
