Responsibilities
- Conduct comprehensive patient assessments and monitor vital signs remotely.
- Coordinate and implement patient care plans in collaboration with the medical team.
- Document patient progress and medical records accurately and promptly.
- Provide emotional support, education, and clear communication to patients and families.
- Manage administrative tasks, including scheduling and referral coordination.
- Ensure compliance with all state health regulations and company policies.
Qualifications
- Active, valid CNA License in Colorado (required).
- Current Basic Life Support (BLS) and ACLS certifications.
- Minimum of 1-2 years of experience in a clinical or nursing setting.
- Strong computer literacy and proficiency with electronic health records (EHR).
- Excellent written and verbal communication skills.
- Ability to work independently in a remote setting.
