Responsibilities
- Conduct comprehensive patient assessments via phone and electronic health records to determine the urgency of care needs.
- Provide triage recommendations to patients, guiding them to the appropriate level of care (urgent care, ER, or primary physician).
- Document all patient interactions and clinical findings accurately and timely in the EMR system.
- Collaborate with physicians and care teams to ensure continuity of care and address complex patient issues.
- Offer patient education and self-care instructions to promote health outcomes and prevent readmission.
- Manage high call volumes efficiently while maintaining a calm, empathetic, and professional demeanor.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in New Jersey.
- Minimum of 2-3 years of nursing experience, preferably in a triage, urgent care, or telehealth setting.
- Bachelor of Science in Nursing (BSN) preferred.
- Strong critical thinking skills and the ability to make sound clinical judgments under pressure.
- Excellent verbal and written communication skills.
- Proficiency in electronic medical record (EMR) systems.
