Responsibilities
- Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and toileting.
- Prepare nutritious meals and snacks according to dietary restrictions.
- Provide companionship and emotional support to clients, reducing feelings of isolation.
- Administer medication reminders and track daily logs.
- Perform light housekeeping and laundry duties to ensure a safe, clean living environment.
- Transport clients to medical appointments and social activities as needed.
- Monitor and report any changes in the client's physical or mental health status to the case manager.
Qualifications
- High School Diploma or GED equivalent required.
- Current Home Health Aide (HHA) or Certified Nursing Assistant (CNA) certification preferred.
- Valid Driver’s License and reliable transportation.
- Must pass a background check and drug screening.
- CPR and First Aid certification (or willingness to obtain).
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a healthcare team.
