Responsibilities
- Provide high-quality direct patient care, assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting.
- Monitor and record vital signs, including blood pressure, temperature, pulse, and respiration rates, reporting any abnormalities immediately.
- Assist patients with mobility, transfers, and ambulation using proper body mechanics and assistive devices to ensure safety.
- Document patient care plans, interventions, and responses accurately and timely in the electronic health record (EHR) system.
- Communicate effectively with patients, families, physicians, and the interdisciplinary care team to ensure continuity of care.
- Maintain a clean, safe, and therapeutic environment for all patients and visitors.
- Administer medications and treatments as directed by the Registered Nurse (RN) or Physician.
Qualifications
- Certified Nursing Assistant (CNA) license required (valid in the state of California).
- Current Basic Life Support (BLS) and ACLS certifications (or willingness to obtain).
- Previous experience in a hospital, long-term care facility, or home health setting is preferred.
- Strong interpersonal and communication skills with the ability to empathize with patients.
- Ability to work flexible shifts, including nights, weekends, and holidays.
- Physical stamina and the ability to lift up to 50 lbs as needed.
