Responsibilities
- Provide direct patient care, assisting with daily living activities such as bathing, dressing, and grooming.
- Monitor and record vital signs, including blood pressure, temperature, and pulse, reporting any abnormalities to the Registered Nurse.
- Assist with mobility, including transferring patients from bed to wheelchair and using proper lifting techniques to prevent injury.
- Prepare and serve meals according to dietary restrictions and patient preferences.
- Maintain a clean, safe, and sanitary environment for all patients.
- Document patient care activities accurately in electronic medical records (EMR).
Qualifications
- Certified Nursing Assistant (CNA) license currently active in the state of Illinois.
- Current Basic Life Support (BLS) certification required.
- 1+ years of experience in home health, hospice, or long-term care preferred.
- Strong interpersonal and communication skills with a patient-centered approach.
- Ability to pass a comprehensive background check and drug screen.
