Responsibilities
- Provide direct patient care and assistance with activities of daily living (ADLs) such as bathing, dressing, and grooming.
- Monitor vital signs and report any changes in patient health status to the nursing team immediately.
- Assist with mobility, transfers, and positioning to ensure patient safety and comfort.
- Administer medications and treatments according to physician orders and established protocols.
- Document patient care accurately and timely in electronic health records (EHR).
- Maintain a clean, safe, and sanitary environment for residents and patients.
- Collaborate effectively with the interdisciplinary team to deliver holistic patient care.
Qualifications
- Current State Certified Nursing Assistant (CNA) license required (valid in WI, IL, or MA).
- Current Basic Life Support (BLS) and CPR certification from the American Heart Association.
- Previous experience in long-term care, assisted living, or hospital settings preferred.
- Strong interpersonal and communication skills with a patient-centered approach.
- Ability to perform physical tasks including standing, walking, and lifting for extended periods.
- Flexibility to work various shifts, including nights, weekends, and holidays.
