Responsibilities
- Provide compassionate patient care in home health and clinical settings
- Assist with ADLs (bathing, dressing, mobility) and vital sign monitoring
- Document care digitally via HIPAA-compliant mobile app
- Collaborate with care teams through remote case management tools
- Support patients with dementia, post-surgical recovery, and chronic conditions
- Maintain accurate health records through our telehealth portal
- Participate in flexible shift scheduling with advance notice
Qualifications
- Active Ohio CNA certification in good standing
- Minimum 1 year acute care or home health experience
- Current BLS/CPR certification
- Reliable transportation for home visits
- Proficient with basic digital health tools (smartphone/tablet)
- Clear background check and drug screen
- Ability to work independently with remote oversight
- Strong communication skills for virtual care coordination
