Responsibilities
- Deliver virtual patient assessments and care coordination via telehealth platform
- Collaborate with interdisciplinary teams to develop and implement personalized care plans
- Document patient encounters accurately in electronic health records (EHR) systems
- Provide health education and support to patients and families remotely
- Monitor patient progress and escalate concerns to clinical supervisors
- Participate in continuous quality improvement initiatives

Qualifications
- Current Illinois RN license (or compact license with Illinois residency)
- Associate or Bachelor's degree in Nursing (BSN preferred)
- Basic Life Support (BLS) certification
- Minimum 6 months clinical experience (externships/internships acceptable)
- Proficiency with telehealth technology and EHR systems
- Strong critical thinking and problem-solving abilities
