Responsibilities
- Provide high-quality virtual patient assessments and care coordination via telehealth platforms
- Monitor patient health data and collaborate with multidisciplinary care teams
- Deliver patient education and support for chronic disease management
- Document accurate clinical information in electronic health records (EHR)
- Participate in remote case management and discharge planning
- Adhere to HIPAA compliance and telehealth protocols
- Contribute to continuous quality improvement initiatives
Qualifications
- Active RN license in North Carolina (or compact state)
- Associate or Bachelor's degree in Nursing
- NCLEX-RN pass result (or scheduled within 60 days)
- Basic computer proficiency and reliable internet connection
- Strong communication and critical thinking skills
- Ability to work independently while collaborating in a virtual team
- Compassionate patient care approach and attention to detail
- Experience with EHR systems preferred (e.g., Epic, Cerner)
