Responsibilities
- Provide remote patient assessments via telehealth platforms and electronic health records
- Develop and implement individualized care plans for assigned patients
- Coordinate interdisciplinary care teams and facilitate patient transitions
- Delicate patient education on chronic disease management and medication adherence
- Monitor patient outcomes and adjust care strategies proactively
- Document clinical encounters accurately and maintain HIPAA compliance
- Participate in virtual case conferences and quality improvement initiatives
Qualifications
- Active RN license in the state of Virginia
- Associate or Bachelor's degree in Nursing (ADN/BSN)
- Basic Life Support (BLS) certification
- 0-2 years of clinical nursing experience (new graduates welcome)
- Proficiency with telehealth technology and EHR systems
- Strong critical thinking and problem-solving abilities
- Excellent verbal and written communication skills
- Ability to work independently in a remote environment
