Responsibilities
- Provide virtual patient assessments via telehealth platform for diverse patient populations
- Administer medications and treatments remotely per physician orders
- Monitor patient vitals and symptoms through digital health monitoring systems
- Coordinate care transitions between in-person and virtual settings
- Document electronic health records with precision and timeliness
- Educate patients on chronic disease management via virtual consultations

Qualifications
- Active Illinois RN license (or compact state eligibility)
