Responsibilities
- Deliver high-quality patient assessments and care coordination via telehealth and in-person visits
- Manage chronic disease management programs for diverse patient populations
- Collaborate with interdisciplinary teams using our integrated EHR platform
- Provide health education and coaching through virtual consultations
- Participate in quality improvement initiatives and outcome tracking
- Adhere to HIPAA compliance protocols in all remote and on-site interactions
- Maintain accurate documentation in our secure patient management system
Qualifications
- Active RN license in Maryland (compact license preferred)
- BSN degree with 2+ years clinical experience
- Experience with telehealth platforms and virtual care delivery
- Certification in BLS and relevant specialty area (e.g., Med/Surg, Telemetry)
- Strong proficiency with electronic health records (Epic/Cerner preferred)
- Exceptional communication skills for virtual patient interactions
- Ability to work independently while maintaining team collaboration
- Flexibility to work rotating shifts including weekends as needed
