Responsibilities
- Assess patient calls and visits using standardized triage protocols to determine urgency and appropriate care level
- Provide accurate clinical guidance over phone and in-person consultations while maintaining HIPAA compliance
- Collaborate with physicians and care teams to facilitate seamless patient transitions through the healthcare continuum
- Document all patient interactions and care plans in the electronic health record system with precision
- Monitor and track patient wait times, ensuring optimal flow through the emergency department and urgent care facilities
- Participate in continuous quality improvement initiatives to enhance triage accuracy and patient satisfaction
Qualifications
- Active RN license in the state of California (CA RN)
- Minimum 2 years of clinical experience in emergency medicine, urgent care, or telehealth triage
- BLS and ACLS certifications with current renewal dates
- Proficiency in electronic health record systems (Epic, Cerner, or similar)
- Exceptional critical thinking and decision-making abilities under pressure
- Excellent communication skills with demonstrated ability to de-escalate anxious patients
- Spanish bilingual proficiency preferred due to diverse patient population
