Responsibilities
- Personal Care: Assist clients with bathing, dressing, grooming, and maintaining personal hygiene.
- Medication Management: Remind clients to take prescribed medications and monitor for side effects.
- Meal Preparation: Plan and prepare nutritious meals based on dietary restrictions and client preferences.
- Companionship: Engage clients in conversation, hobbies, and light activities to promote mental well-being.
- Light Housekeeping: Perform light cleaning tasks such as laundry, dishwashing, and changing linens to ensure a safe environment.
- Documentation: Accurately complete daily care logs and progress notes in compliance with company policies.
- Vital Signs: Monitor and report changes in client health status to the RN Case Manager.
Qualifications
- Education: High School Diploma or GED required; CNA or HHA certification preferred.
- Licensure: Current Texas State CNA/HHA license or eligibility to obtain one.
- Experience: Previous experience in home health or geriatric care is highly desirable.
- Driver's License: Valid, clean driver's license and reliable transportation required.
- Background Check: Must pass a criminal background check and drug screen.
- Communication: Excellent verbal and written communication skills with a compassionate demeanor.
- Physical Stamina: Ability to lift, bend, and stand for extended periods as needed.
