Responsibilities
- Perform comprehensive patient assessments via phone and in-person to determine acuity and urgency levels.
- Provide expert medical triage advice and self-care instructions for non-emergent conditions.
- Document all patient interactions, assessments, and care plans accurately within the Electronic Health Record (EHR).
- Coordinate referrals to physicians, specialists, or urgent care facilities as needed.
- Educate patients on preventative health measures and chronic disease management.
- Collaborate closely with physicians and nursing staff to ensure continuity of care.
- Maintain high standards of patient satisfaction and safety protocols.
Qualifications
- Active, unencumbered Registered Nurse (RN) license in the state of Indiana.
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Minimum of 3 years of progressive experience in triage, urgent care, or emergency nursing.
- Proficient in using EMR/EHR systems (e.g., Epic, Cerner, AthenaHealth).
- Strong analytical and critical thinking skills under pressure.
- Excellent verbal and written communication skills.
- Knowledge of ICD-10 coding and medical terminology.
