Responsibilities
- Perform comprehensive initial assessments of patient symptoms via phone and in-person triage.
- Prioritize patient care requests based on clinical urgency and acuity levels.
- Evaluate patient complaints to determine appropriate level of care and referral pathways.
- Document all triage interactions accurately and promptly in electronic health records (EHR).
- Collaborate with physicians, advanced practice providers, and care coordination teams.
- Provide patient education on self-care management and preventative health strategies.
- Monitor call queues and manage high-volume patient interaction periods effectively.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in New York State.
- Graduate of an accredited Nursing program (Associate, BSN, or ASN).
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Strong communication skills with the ability to de-escalate difficult situations.
- Proficiency in using computer systems and navigating electronic medical records.
- Ability to work in a fast-paced, high-volume call center environment.
