Responsibilities
- Provide direct patient care, assisting with activities of daily living (ADLs) such as bathing, dressing, and grooming.
- Monitor and record patient vital signs, including blood pressure, temperature, and pulse, accurately and in a timely manner.
- Assist with medication administration under the direct supervision of a registered nurse.
- Communicate effectively with patients, families, and the interdisciplinary healthcare team to ensure holistic care.
- Maintain a clean, safe, and comfortable patient environment, adhering to infection control protocols.
- Document care activities and patient observations promptly in electronic health records (EHR).
- Transport patients to and from appointments or therapy sessions as needed.
Qualifications
- Certified Nursing Assistant (CNA) license required, valid in the state of Texas.
- Current Basic Life Support (BLS) certification.
- Strong interpersonal and communication skills with a patient-centric approach.
- Ability to stand for extended periods and lift up to 50 lbs safely.
- Previous experience in home health or long-term care is preferred.
- Valid Texas driver's license and reliable transportation.
- Ability to work flexible hours, including evenings and weekends.
