Responsibilities
- Provide high-quality patient care via in-person visits and telehealth platforms
- Monitor patient vitals and administer medications according to protocols
- Collaborate with interdisciplinary teams to develop personalized care plans
- Document patient records accurately using EHR systems
- Educate patients on health management and preventive care
- Respond to patient inquiries through secure remote channels
- Adhere to HIPAA compliance and healthcare regulations
Qualifications
- Active California RN license
- Minimum 2 years clinical nursing experience
- Bachelor of Science in Nursing (BSN) preferred
- Experience with telehealth platforms or remote patient monitoring
- Strong proficiency in EHR documentation systems
- Valid BLS/CPR certification
- Ability to work flexible shifts including evenings/weekends
- Exceptional communication and problem-solving skills
