Responsibilities
- Provide virtual patient assessments and care coordination via telehealth platforms
- Administer medications and treatments per physician orders remotely
- Monitor vital signs and document health data accurately in EMR systems
- Collaborate with interdisciplinary teams for comprehensive care planning
- Offer patient education on chronic conditions and treatment adherence
- Maintain HIPAA compliance in all digital communications and record-keeping
- Respond to patient inquiries and triage urgent situations appropriately
Qualifications
- Active Texas CNA or RN/LPN license with unrestricted status
- Minimum 1 year clinical experience in home healthcare or telehealth
- Proficient with telehealth technology (Zoom, Epic, etc.)
- Strong written communication skills for digital patient interactions
- BLS/CPR certification (current)
- Ability to work independently with minimal supervision
- Reliable high-speed internet and dedicated home workspace
- Valid Texas driver's license (for occasional in-person visits)
