Responsibilities
- Deliver remote patient assessments and care coordination via telehealth platform
- Monitor and evaluate patient health status through digital health records
- Provide clinical guidance to patients and family members via video consultations
- Collaborate with interdisciplinary care teams for holistic treatment plans
- Document patient encounters accurately in EHR systems
- Respond to urgent patient inquiries and triage situations remotely
- Participate in continuous quality improvement initiatives
Qualifications
- Active Ohio RN license with current BLS certification
- Minimum 2 years clinical nursing experience
- Proficiency with telehealth technology and EHR systems
- Strong critical thinking and assessment skills
- Excellent communication and empathy for remote patient interactions
- Ability to work independently in a virtual environment
- Experience with chronic disease management preferred
