Responsibilities
- Conduct comprehensive virtual health assessments via telehealth platform
- Provide patient education on chronic disease management and medication adherence
- Collaborate with multidisciplinary teams to develop personalized care plans
- Document clinical encounters accurately in electronic health records (EHR)
- Triage urgent patient calls and coordinate appropriate care interventions
- Monitor patient progress through virtual follow-ups and health data analysis
- Adhere to HIPAA compliance and telehealth best practices
Qualifications
- Active RN license in Minnesota (unrestricted)
- Minimum 2 years of clinical nursing experience
- Telehealth or virtual care experience preferred
- BLS/CPR certification (AHA preferred)
- Proficient with EHR systems and telehealth technology
- Strong critical thinking and decision-making abilities
- Excellent written and verbal communication skills
- Ability to work independently in a remote environment
