Responsibilities
- Conduct comprehensive patient assessments via phone and secure messaging channels to determine the acuity of medical conditions.
- Apply critical thinking and clinical protocols to triage symptoms, recommend appropriate care pathways, and facilitate appointments when necessary.
- Document all triage encounters accurately, timely, and thoroughly in the Electronic Health Record (EHR) system.
- Provide expert health education, medication instructions, and self-care recommendations to patients and caregivers.
- Collaborate effectively with physicians, nurse practitioners, and care managers to ensure continuity of care.
- Manage high-volume inquiries while maintaining a calm, professional, and empathetic demeanor.
- Stay updated on clinical guidelines, telehealth best practices, and regulatory compliance standards.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the State of Texas.
- Minimum of 2 years of recent experience in acute care, emergency room, or triage nursing preferred.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Proficiency in navigating EMR/EHR systems (e.g., Epic, Cerner, AthenaHealth).
- Excellent verbal and written communication skills with a focus on patient-centered care.
- Strong computer skills and ability to work independently with minimal supervision.
- Ability to handle complex patient scenarios with high emotional intelligence and professionalism.
