Responsibilities
- Assess patient calls via telehealth platform to determine medical urgency
- Provide evidence-based clinical guidance and health education to patients
- Collaborate with providers to coordinate care transitions and follow-ups
- Maintain accurate documentation in EHR systems while adhering to HIPAA protocols
- Triage emergent cases to appropriate emergency services or providers
- Manage high-volume call queues with efficiency and empathy
- Participate in quality improvement initiatives and training programs
Qualifications
- Active California RN license in good standing
- Minimum 3 years clinical experience with ER, urgent care, or telehealth background
- BLS certification; ACLS preferred
- Proficiency in Epic or similar EHR platforms
- Exceptional critical thinking and communication skills
- Ability to work independently with minimal supervision
- Experience with telephone triage protocols (TDS, ESI)
- Spanish bilingual certification highly desirable
