Responsibilities
- Patient Triage: Utilize advanced clinical judgment to assess patient symptoms via phone and secure messaging to determine the appropriate level of urgency.
- Care Coordination: Facilitate timely referrals to physicians, specialists, or urgent care facilities based on triage assessments.
- Documentation: Accurately document all patient interactions, symptoms, and care plans in electronic health records (EHR) while strictly adhering to HIPAA guidelines.
- Health Education: Provide clear, empathetic instructions to patients regarding self-care, medication management, and when to seek emergency services.
- Workflow Optimization: Collaborate with the care team to streamline patient flow and reduce unnecessary emergency room visits.
Qualifications
- Licensure: Current, active, and unrestricted Registered Nurse (RN) license in the State of Arizona.
- Experience: Minimum of 2 years of nursing experience, with a strong preference for background in triage, urgent care, or telehealth nursing.
- Education: Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) is highly preferred.
- Certifications: Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Communication: Superior verbal and written communication skills with a focus on empathy and conflict resolution.
