Responsibilities
- Conduct comprehensive patient assessments and triage via telehealth platforms to ensure optimal care delivery.
- Administer medications and treatment plans remotely in accordance with state licensure and facility protocols.
- Document patient interactions, medical history, and treatment progress accurately in electronic health records (EHR).
- Communicate effectively with physicians, specialists, and interdisciplinary teams to coordinate seamless patient care.
- Provide patient education and counseling to promote health literacy and self-management of chronic conditions.
- Monitor patient outcomes and escalates critical issues immediately to ensure patient safety and compliance with regulatory standards.
Qualifications
- Active, unencumbered Registered Nurse (RN) license in the State of Texas.
- Minimum of 1-3 years of clinical nursing experience (acute care or telehealth preferred).
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications are required.
- Proficient in using computer systems, EHR software (e.g., Epic, Cerner), and video conferencing tools.
- Strong communication skills with the ability to build rapport with patients remotely.
- Ability to work independently in a remote environment with high levels of self-discipline.
