Responsibilities
- Provide direct patient care, including monitoring vital signs, administering medications, and assisting with daily living activities.
- Assess patient needs and develop individualized care plans in collaboration with the nursing team.
- Document patient care accurately and promptly in electronic health records (EHR).
- Respond to patient emergencies and alarms with urgency and professionalism.
- Collaborate with physicians, therapists, and social workers to ensure holistic patient care.
- Maintain a clean, safe, and therapeutic environment for patients and staff.
Qualifications
- Current, unencumbered RN or CNA license in the state of Illinois.
- Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certification required.
- Minimum of 1 year of experience in a hospital or long-term care setting preferred.
- Strong communication and interpersonal skills.
- Ability to stand for extended periods and lift up to 50 lbs.
- Compassionate demeanor with a focus on patient-centered care.
