Responsibilities
- Provide comprehensive patient assessments and triage services via secure video conferencing and telehealth platforms.
- Develop and implement individualized patient care plans in collaboration with physicians and the interdisciplinary care team.
- Document patient interactions, vitals, and clinical notes accurately and timely within the Electronic Health Record (EHR) system.
- Monitor patient health status and escalate critical concerns to providers immediately to ensure patient safety.
- Educate patients and their families on disease management, medication adherence, and preventative health strategies.
- Collaborate with other healthcare professionals to ensure continuity of care and seamless transitions for patients.
- Maintain strict adherence to HIPAA regulations and hospital privacy policies while handling sensitive patient information.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the District of Columbia (or compact state eligibility).
- Minimum of 2 years of clinical nursing experience in a hospital or acute care setting (Telehealth experience preferred).
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications are required.
- Proficient computer skills with the ability to navigate electronic health record systems (Epic, Cerner, etc.) and video conferencing software.
- Strong verbal and written communication skills with the ability to build rapport with patients remotely.
- Ability to work independently and manage time effectively in a remote work environment.
