Responsibilities
- Conduct comprehensive phone and digital patient assessments to determine the urgency of medical conditions.
- Apply clinical judgment to prioritize patient care and facilitate appropriate referrals to physicians, specialists, or urgent care facilities.
- Provide accurate medical advice, health education, and self-care instructions to patients and families.
- Document all patient interactions, assessments, and care plans accurately in the Electronic Health Record (EHR).
- Collaborate effectively with providers, nurses, and ancillary staff to ensure seamless patient flow.
- Monitor trends in patient calls to identify system-wide improvements and educational needs.
Qualifications
- Current, active Registered Nurse (RN) license in the State of Indiana.
- Minimum of 1-2 years of experience in triage, urgent care, or a similar clinical setting.
- Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Strong critical thinking and decision-making skills under pressure.
- Excellent verbal and written communication skills with a focus on patient-centered care.
- Proficiency in navigating electronic medical records and documentation systems.
